I was asked for my last 4 digits of SS# but just to set up a portal with my hospital system in order to sign up for a vax appt.
My neighbor went to a pharmacy for her vax, and no SS# was req'd.

@Shelbelle The ethnicity question helps to capture data to insure minority groups are equally receiving the vaccine.

Yes, my DIL filled out my papers,while we sat in the car. she asked for it. I didn't care was glad to get my vaccine. I did once say at a DR. office, my #SS is if I want a job and I don't want one here, I already work at the hospital. Never asked again. I don't know that it's that much of a secret, but annoying to fill out papers every time you see a DR.

I always ask why paperwork or individuals need my SS#---if it doean;t seem right to me--I never put it down. the main entities in my life aready have it.

When we got our call to schedule our first shot, the man (reading from a script) asked us for our SS#. We told him that we didn't feel comfortable giving him that, he said ok and scheduled our time. No biggy.

i just think its TMI! who the heck knows what database these forms go in! Especially this form it's hosted by Go-Daddy!
Then dont get it! There WAS a time when your medicare # WAS your Social Security # followed by an "A" or a "B"...No one is forcing you to get the vaccine or to get it at that location-
@Mothertrucker my Medicare card uses my SSN. I started on Medicare before the change in identifier.
Vaccines are scarcer than hens teeth in my state. was just sh*# luck that there was an appt available.
Edited: called Medicare and they gave me the generic number. guess when the cards were updated never received a replacement card and they're sending a new Medicare card.

It's not any kind of requirement -- legal or otherwise-- to give one's SS# to get the vaccine. You cannot be denied for not giving it.... and I wouldn't. I would just leave such a question blank. In fact I distinctly remember a news report saying that people should not give their SS# to any health official or facility for the purpose of getting the vaccine.

I'm not who you paged, but to answer your question from my experience:
When I called to set up the initial vaccine appointment I was told I would only need identification (driver's license), no insurance needed.
When I arrived at the vaccine clinic site I had to fill out the Pfizer disclaimer forms, including my name, address, birthdate, and ethnicity. Nothing else. I was never even asked for any form of I.D.
When issued by the state, our vaccines were completely free of charge. When issued by an independent pharmacy, we were told that they may charge your insurance for administrative costs, but if you did not have insurance you still would not be charged anything.

@gertrudecloset,Walgreen's doesn't need your ssn if you already have an account with them for prescriptions. They have access to your health insurance information and can tell your status by the health insurance information on file.
What I'm trying to say is: I may get my shots from a location that knows nothing about me. I used the drug store as an example. In my state, we have a lot of choices to choose from ~ it's just a matter of waiting it out for your turn. Of course, Walgreen's has my prescription information, but what does that have to do with my vaccine? My prescription insurance won't shed light on my identity in terms of a social security #. All the drugstore does is fill scripts that come from my doctor. That's it. They do not have my personal medical information. They really don't @pitdakota . They can see which meds I've have been using but that's it.
My drugstore does not have my health insurance information @pitdakota . I don't know how many ways I can say this.
