I've had both shots and was asked for Drivers License # and last four of SSN. This is necessary information since there is going to be a master vaccination list and this information will also be used for vaccine passports that you will need to travel internationally and, who knows, domestically. I was glad to give the required information; I think some of us are unduly paranoid.

Then dont get it! There WAS a time when your medicare # WAS your Social Security # followed by an "A" or a "B"...No one is forcing you to get the vaccine or to get it at that location-

Hi @patbz this doesn't appear to be working as every State and/or municipality isn't asking for it (as you can see from the responses).
If they ask me for mine when I do get my shots, they won't get it. I'll give them my Medicare info, but that's it! There is a reason your social security # is no longer your Medicare I.D. #. For those who don't have Medicare yet, I don't know....but I'm leary about my personal information in yet another database...

Who said it was "free" ??! It is an expensive vaccine- The person receiving the vaccine is not CHARGED for it-- that is not even close to being the same thing as "free"..

It is the state that needs to keep the data bank & information for those individuals vaccinated. The reason is quite simple. Just in the off chance there is a problem determined to be inefficient or something wrong with a lot number of a certain vaccine, they have a record to contact all individuals who received a vaccination with that particular vaccine with that lot number.
They do not rely on the individual to keep the record of the vaccination in order to contact them. And probably a good thing. Can't tell you the number of people that have arrived for their 2nd vaccination that don't know what they did with their card or paper from the first vaccination.

@gertrudecloset, okay. What I'm sensing is the difference between free to us and free in general. So yes, someone pays, but it is not the person getting the vaccine. If that person is billed, it is against the CDC rule.

It is more a reason for record keeping to keep track of the doses and where the money needs to go; either to the state or the federal government. Then the state needs to track to what program where they are have spending their funds. Accounting experts would be better able to explain that process than I can.
Operation Warspeed purchased a relatively small amount of vaccine from certain manufacturers. Then other legislation, including the American Recovery Act purchased millions more doses of vaccine from the manufacturers. So it is paid for out of those pieces of legislation. But they still have to do accounting and bookkeeping for where the vaccine is disseminated; either the state or the federal government..
Some of that money in the legislation will be placed toward the federal government to cover the cost of vaccination for veterans, federal prisons,some federal government employees, etc.
The federal government is in charge of the long term care vaccination program, federal prisons, and all veteran hospitals and veterans domiciliaries. Depending on who administered the vaccine, that agency receives the money. Obviously the veterans administration does not oversee the budget of the federal prison system or other federal agencies, so they have to track it to see who gets the money.
The state oversees the remainder of the vaccination process and they have to determine how much goes to the various pharmacies, public health departments that are doing mass vaccinations, hospital agencies, and other vaccination programs that administer the vaccine to residents of the state.
