I signed up using My Chart as others have done and while they did ask for the last four digits of my SSN they also indicated that giving them wasn't mandatory, so I opted not to. It didn't matter.

I signed up today for later this week. They did not ask for Ss# or last 4 digits, but they did ask for mother’s maiden name.  I’m in PA.  Name, address, a few health questions and insurance info.   I hear it’s no charge, but I’m guessing if you have insurance they may bill for the vaccine, fee to dispense or both.  A no insurance option was a choice. I’m curious about it. They did ask race and gender also, which you could choose prefer not to answer.

Did she go ahead an provide it?  Otherwise, I would tell her NOT GIVE OUT HER SOCIAL SECURITY #.  There is no need for this information.

Lot's of places ask for SS# that are not necessary anymore.  Do not give this info out.

Then it wasn't really free if you got it through your employer was it?  Even though employer is Federal government? @Silver Lining Your situation a little different maybe.

Then it's really not free to you.  Your Medicare Supplemental Insurance is going to cover it @Jaspersmom .  Don't you pay a premium for Medicare SI?

@CatsyClineIts not necessary information.  Reminds me of employers and doctors offices asking for your important info to be sitting around in their files when they don't need it.

I don't understand how anyone can be charged for the vaccine. It is totally free.

From the CDC;

The federal government is providing the vaccine free of charge to all people living in the United States, regardless of their immigration or health insurance status.

COVID-19 vaccination providers cannot:

• Charge you for the vaccine
• Charge you directly for any administration fees, copays, or coinsurance
• Deny vaccination to anyone who does not have health insurance coverage, is underinsured, or is out of network
• Charge an office visit or other fee to the recipient if the only service provided is a COVID-19 vaccination
• Require additional services in order for a person to receive a COVID-19 vaccine; however, additional healthcare services can be provided at the same time and billed as appropriate

COVID-19 vaccination providers can:

• Seek appropriate reimbursement from the recipient’s plan or program (e.g., private health insurance, Medicare, Medicaid) for a vaccine administration fee
  • However, providers cannot charge the vaccine recipient the balance of the bill
• Seek reimbursement for uninsured vaccine recipients from the Health Resources and Services Administration’s COVID-19 Uninsured Programexternal icon

@gertrudecloset   I guess I’ve only heard no charge to patient.  No charge to patient doesn’t necessarily mean free.  Some reports saying free, some reporting no charge to person being vaccinated.  Hard to believe anything.......
