I signed up using My Chart as others have done and while they did ask for the last four digits of my SSN they also indicated that giving them wasn't mandatory, so I opted not to. It didn't matter.

I signed up today for later this week. They did not ask for Ss# or last 4 digits, but they did ask for mother’s maiden name. I’m in PA. Name, address, a few health questions and insurance info. I hear it’s no charge, but I’m guessing if you have insurance they may bill for the vaccine, fee to dispense or both. A no insurance option was a choice. I’m curious about it. They did ask race and gender also, which you could choose prefer not to answer.

Lot's of places ask for SS# that are not necessary anymore. Do not give this info out.

@CatsyClineIts not necessary information. Reminds me of employers and doctors offices asking for your important info to be sitting around in their files when they don't need it.

I don't understand how anyone can be charged for the vaccine. It is totally free.

@gertrudecloset I guess I’ve only heard no charge to patient. No charge to patient doesn’t necessarily mean free. Some reports saying free, some reporting no charge to person being vaccinated. Hard to believe anything.......
