Many times I have ordered a TSV or a OTO. When it arrives, it's defective. Of course QVC can't exchange it because it has sold out or is not available in the color I want. So they refund the money. A few months later it shows up at a much higher price. If I REALLY want it I have to pay the higher price.
It's too bad they don't have a system that if you returned a defective item, they could offer you the lower price when the item came back in stock - if it did. As for me, I have never paid more than I originally paid to order an item. I suppose I would if I REALLY wanted it. And I guess it would be hard for them to keep track of every defective return.
I'm getting ready to return VPH's fountains that were a OTO. I ordered two. One is perfect, and is now working beautifully. The other leaked all over the floor. Naturally they're sold out of the color I want, so they will probably refund my money. I'd really like to have this, so I'll have to wait until it comes back into stock and pay a higher price. 
Does this bother anyone else?

They've adjusted the price for me in a similar circumstance to what you've described.

I must be very lucky...I have received the wrong merchandise 2x..other then that...no problems.

It didn't happen to me but I often think of that when they say oh its returnable for 30 days. Or now with the Christmas in July stuff. that's guaranteed till the end of Jan 2015. I can tell you that I bought lots of things over the years on Q that were no longer available when I went back months later to type a comment OR to reorder. So them saying you can return it yes you can, but don't be so sure youre going to get the SAME items in exchange! And IMO they should STATE THAT.

if something is part of a set and only one item is damaged or missing i usually make a phone call to get a credit for the damaged/missing item and keep the working item. sometimes they dont even want the damaged item back.

kachina - You're very lucky it's never happened to you! Lately I've had a few, but I guess it really isn't that bad - it just seems like it is!
I ordered TSV earrings for a gift (not CP). The post on one was at the top, and the post on the other was in the middle. I tried them on. They were uneven on me, so I know they'd be uneven on her. I ordered two fountains. One was fine; the other wasn't. The color I ordered is gone. I PRE ordered an Issac bag that was a TSV. It hadn't been presented on air yet. It was a return. There was a huge scratch across the middle and the zipper didn't work. The color was gone. At Christmas, I ordered Halo TSV's as gifts. One was purple leopard. They sent me a Joan Rivers purple leopard scarf! The TSV purple leopard was sold out. I ordered a Mally foundation OTO in Fair. The invoice said "Fair" but the bottle said "Deep"! The Fair was sold out.
If it happens again, I'll call and see if they will adjust the price. I really like the fountain, but the color is gone. They'll refund my money, but if they eventually get it back in it will be at a higher price - which I shouldn't mind paying if I really want it, and I really do!
