There may be some instances where hot car deaths are intentional.  I believe in most cases it is a tragic accident, caused by a caregiver who is not routinely responsible for the transporting of the child. People are creatures of habit and can be tripped up if their routine is broken. I will never forget an episode of Oprah where she had parents who had accidentally caused the death of their children and it was horrifying to listen to their stories.  The depth of their grief and contrition was palpable. The hell they live with is more punishment than serving a life sentence in prison.
