I haven't ever had a dog do this. My brother's dog is. He will find out at noon what's up with that. He's already had med's, to no avail.. I've looked it up, brother thought it was sneeze that made the head hit the ground and the nose bleed.
But the vet is talking transfusion, possibly being needed. I just don't know.
