I've always used a flat/top sheet for comfort and hygiene reasons.
Even if I don't use anything else on it.

Being that my sheet sets contain a top sheet, I use it. Never got tangled in a sheet. On warm nights, the sheet is sufficient for warmth.
Speaking of lazy, when I was school age, I had a light weight sleeping bag, and I slept in it on top of the bedspread.

If you have no top sheet then you are getting your comforter or duvet or whatever you cover yourself up with, dirty. And that has to be washed often.
What concerns me are hotel beds. You just know they don't wash the comforters in between customers so without a top sheet I'd be wrapped up in someone else's bodily cooties, just gross.

I always use top sheet even growing up. Of course I change my sheets every week and shower everyday. My mother was a stickler I even make my bed everyday -- guess old habits stay with us!

My grandbabies (12 and 14) don't use them. I don't need one, but since hubby wants one, it's on our bed.
My daughter doesn't use one either.
I agree that a top sheet is old school.

I wash my quilt on the same day I wash my sheet/pillowcases. It's a king size, but thin and soft, and fits easily into my front load washer. I haven't used a top sheet in several years, and prefer it that way.
I'm more often cold than hot, but I have a faux fur throw that I use on top of the quilt, if I'm too cold.
Just because you and others do things one way @amyb, it doesn't mean it's the only way they should be done! It doesn't make us dopey, just different.
