I've had this happen at Amazon, too. Once I paid myself to return, rather than use UPS label and wait for them to come get it. My return went through and received my refund so I threw away myn USPS receipt. Two months later they said they didn't get my returned merchandise and charged me for it all over again! I hate Amazon still for that.
I just send back the small part of the receipt that has the reason for return, etc., keeping the top part (with labels affixed on the back) for my records, so there's no way it's scanned by 'accident'.
I think it's more likely to be lack of paying attention to whether or not the Q label is used.
Another reason I don't use Q's label is they just scan the package to be opened at a later date and I have had a problem with not getting credit when there was more than one order in the package, but only 1 return label used.

I've noticed lately that more and more places are beginning to pay for the return shipping. I think it might become an ongoing trend with people doing so much more online shopping and so many stores in malls closing.
I read on here that if you pay for a purchase with Paypal, they will refund the return shipping for you once a month. I haven't tried it yet.
A lot of times when I order on Amazon, they say they will pay return shipping. My mother often orders through various small catalogs she gets in the mail because she wears such a tiny size (petite extra short). They often have free shipping and pay return shipping.
