@Kachina624 @Sorry I wasn't clear. He only needs to get dr to help renew his placard but the form that's required is new. Prior to this, once deemed permanent disabled by two doctors, he only had to renew once, years ago. I think this is coming about because there are so many people in Florida with disability parking permits. The form that just came only allows placards for 8 reasons. Looks like the state is trying to tighten things up, but now is not the easiest time. If I knew the firm was in the envelope behind the license plate renewal form, my DH could have taken it with him went he went in to see the doctor. I didn't even know the extra form was there until I went to pay for the license tag renewal today. I'm sure it will work out fine but it's just such a hassle for disabled people to do this., especially right now.

Sorry to say that this form did not come separately from the tag renewal; it was in the same envelope. We didn't know it was there, or that this new requirement existed. My husband has not had to fill out any forms for renewal of his parking permit. This is something new and it came with no warning.

@Mindy D I'm not quite following. Most states, including FL, require renewals every 4 years on the recipients date of birth for permanent tags.
Is you DH's birthday coming up?
I think the form is confusing you. Go to the website and read what you need to do with it and see if the form on the website is the same as the one you received.
DS and DH both have the blue hang tags so I understand your concern.
You don't have to get into detail but if your DH has had his tag for 4 years it is time for renewal, and if he has had it longer then at some point you must have had to renew.
Those forms usually can be signed and sent in but if you drive I would deliver it in person with the expiring tags. Your doctor I'm sure is aware of what to do.
I don't this is thoughtless as much as it is standard assuming people who have tags know what to do. Your website-I couldn't find anything that was new; the last update was 2018.
Good luck; it's frustrating and I'm sorry your DH is disabled.

Yes, his birthday is coming up. Maybe I never realized he's had to do this, this way, before. He's had his tag much longer than four years. We understand the form. Thank you.

@Mindy D Too bad your doesn't do what ours did. All tags and licenses are forgiven from April til October.
Gives you a real thrill to be a lawbreaker. Still have the old lady card too 🧑🏻🦳

I would scan the form and send it to the doctor as a PDF.

@Kachina624 Not sure if it's the same in all states but my friend in MI is permently disable so was able to get a perment sticker for her plate but also needs a plackerd so if someone drives her to the Doctors it can be used. She got a noticication she needs to go to MV and have new form fron Doc. Think I got this all right but sounds crazy to me
@mspatmac @Mindy D So if you have a placard, why not just use that and forget the license plate? That's what I do.

@Kachina624 I can only speak for my friend but she kept forgetting the placard in her friends car, so she had nothing on her car when she wanted to use it. I know she should tie a string around her finger but she would forget what it was for. LOL

Maybe the requiremets are different in your state, but a separate visit to your doctor usually is not required. I'd call whichever doctor is needed for the form (usually it's the primary physician) and tell them you need a new authorization. The last time this was required, I dropped off the form to his doctor for him to complete. He did that and mailed it back to us. If necessary, I would have driven back to his office to pick up the completed form.
I know it's a pain in the neck, but placard renewals are common after a certain number of years. Could you maybe consider just getting disabled license plates instead of using the placards? We finally did that and it sure is a lot easier. Best of luck to you.

Yes, @Mindy D is correct. Last year FL changed the "disability" requirement to obtain handicapped placards, prior it was "permanent" or every 5-6 years if I recall.
Spouse received new form to be completed by his doctor w/new regulations last year (2019) the month of his bday.
I think everyone and their brother has "handicapped" placards in FL. I questioned DMV as his placard was good until 7/22. I was told to "weed out" those who didn't need it & doctor needed to fill out again.
His primary physician told us he was no longer filling out disability placard forms although he was the doctor who originally signed the forms. He told him he needed to go to his specialist (respiratory doctor). The specialist would not "sign the form" and referred him back to primary (they hold doctor who signs responsible) . Took almost a month to get it resolved.
Had it issued after all this by primary doctor🙄. Went to DMV and was issued 4 year placard. There is no "time limit" to apply but I'd definitely start now if your doctors are anything like his!!!
(I had to go to DMV office today to pay my registration which is tax collector also. Normally, it's a quick & efficient less than 1/2 hour. Today, there were 60+ people in line -- waiting to enter, it was 90+ degrees today, 😷 and social distancing ... no way was I getting in that 2 hour wait line -- I mailed my registration and paid the extra fee!)
