I've been shopping with QVC since 1980 and when ever I return something, I've never gotten a used item.
Then again I have my own method for returning.
I never mark the return / invoice part of the reciept "replace with same item" or "exchange with same item in the following:....."
I always mark the return / invoice part of the receipt "refunded: by original method of payment".
Then I order the same item in the size or color that I need. Why?
I never know how long it will take to replace or exchange and, I don't know where that replacement is coming from.
When I order the same item myself, I know I'll get my item from the warehouse where the new items are stored.
I use the same method with brick & mortor stores also. I just return the item for a refund and go back later or have my friend purchase a new one off the rack.
