Cats with Hot Spots

I will be taking my boy cat back to the vet today because of a very raw hot spot on his back leg. He started getting these about five years ago at nine- month intervals. He'd get a shot and the spots would heal up. But now the flares are more often and the last shot didn't do its magic. I changed his diet to no grains but that obviously isn't the prob. Has anyone found a solution to these things?
