Anyone else still waiting for the KA Grinder Attachment?

I ordered the KitchenAid stand mixer the first of November when it was the TSV. Apparently, the form for the free Grinder Attachment (aka a rebate) wasn't included inside the box like the hosts all said it would be.

I called CS and they said they were aware of the problem and mailed me the form about 10 days later. I filled it out per the instructions and included the original "proof of purchase" from the shipping box.

Here it is now, 2 months later and I'm still waiting for my Grinder. Anyone else still waiting?
