I just finished Ms. Bixby's Last day by John David Anderson.  It is recommended for Grade 4 through 6.  But I think it is more for Grade 6 and up.  Ms. Bixby is a six grade teacher who really connected with a group of three boys. And those three boys really went out of their way to try to make her feel good as she undergoes cancer treatment in the hospital. It is a very touching story.  I highly recommend it, especially if you have a middle schooler.  I personally really enojoy it, too, and I am way older than the recommended age.
