I get a spot on my upper lip, mine has no color just a dry patch. I use gentle acids only face and that seems to keep it away. If I stop using acids it appears back pretty quickly.

Treat your spot only, since you have sensitive skin. But, I would suggest a more mild AHA, lactic acid.
@AngelPuppy1 I also wanted to suggest MAP vitamin C, or even the THD formulation for your skin overall.

I have such sensitive and reactive skin -- does anyone have a suggestion to soften this thing up? I've tried everything I can think of!

Please see a dermatologist. It sounds like it could be a keratosis which I have had many of and they are removable. But I also have had basel cell skin cancer twice so I have every odd spot checked out! Do not ignore this!
