Looks like it just the number of speeds--Mia 2 has 2 speeds and the 3 has 3 speeds. and of course there is a price difference too. I have the Mia and it does me very well. Not sure way you need anymore than 1 speed. Got mine at Nordst*rom for under $80 on sale a couple years ago.

The number of speeds, the charger (Mia 3 comes with a stand AND a P-Link), and the Mia has a light that indicates the charge level. If you go to the Clarisonic website, you can watch videos of each and learn more about the differences. My Clarisonic Plus broke in 2014 and I've been waiting for a Mia 3 TSV for ages with no luck.
