I had no problem getting credit cards in the late 70's. As for when I had my tubes tied in 1992 I was 35 no kids and my doctor did it with hesitation. Husband didn't have to sign anything. My choice. He would have signed it anyway if need be but it wasn't an issue.

It was around 1973-74 women won the right to get credit cards and loans.  It was before that they need a male signature.  

By 1992, around 20 years later, women had the same rights as men.

My DH had a vasectomy in 2000. I had to sign my consent

That surgery center is not normal.

I'm retired and have been around when  6 siblings, their spouses/ kids, my kids/spouse, and my parents had procedures/surgeries in many parts of the country during several decades. No spouse ever had to sign off for another.  

I didn't have to sign off when my husband had a vasectomy or any other procedure/surgery. Likewise, he never had to sign off when I had my tubes tied or any surgery/procedure.
