I cannot imagine telling a person if they need to use the bathroom to go a half mile to a service station or fast food restaurant. You trust this person to work on your property but not to use your bathroom. I find that odd and rude.

Your house...your rules. I have found it is always best to go with my 'gut' feelings.
I would think it would also depend on the # of bathrooms one has. If your only one contains prescription drugs I would hesitate. I have a 1/2 bath on the main floor with just the basics...sink,toilet,&hand towels/paper towel roll( I like to give a choice to guests not wishing to use a hand towel that others have used).
Most companies that we have hired have probably informed thier employees to not ask, but go to a nearby business. ) I live in an urban area so we are very close to easy access public restrooms.

I don't ever remember a worker outside asking to come in to use the rest room. Someone working inside the house have asked to use it and that is perfectly fine.

As a general rule, of course I'd allow someone I'd contracted with to perform work around my place to use the bathroom, If I had bad vibes, the entity for which he or she worked was a complete unknown or I felt uncomfortable for some reason, I'd wait near the door until they finished and left. Expecting them to get in their vehicle and drive to a service station or fast food outlet seems extreme to me.
In the OP, it was noted 'we' refused. Were I not alone in the house I'd be even less likely to decline to permit a worker to use the bathroom. Of course, safety is important but this smacks of elitism,

Only once I had a repair person ask to use the bathroom..They were here for hours I have a half bath in my laundry room..So that was OK..never upstairs..I had one guy say he "watered" the trees in my woods..I thought that was disgusting.
