was he trying to effect a gender change with this electroshock treatment and those injections? They made the discussion so vague that I wasn't really sure what he was attempting to accomplish.

Not Robert, Thomas (Barrow.) He is gay and is trying to find a "cure" for it. Was taken by some quack remedy (the electroshock therapy and the injections) and had to seek real medical help when the injection site became badly infected..

And slightly O/T did anyone see the tiny box that Bates found in their home with Anna?

I never heard of that 'device' until I read it on one of the DA websites as to what it was.

I thought Anna was buying some French letters for Lady Mary in a previous episode.

From what I read about the "device", it was a sponge .... and there was a liquid to moisten the sponge as a spermicide ..... often the liquid was olive oil.

That's all I remember from the article.

I'm glad Anna didn't tell Bates about Mary's little jaunt.

By the way I have been reading that Season 6 of DA will be the last. Julian Fellow is hard at work on a new series and almost all the cast has fulfilled their contracts and are now receiving much more lucerative offers for movies and such and they want to be free to do other things.
