I just received the results of a medical test I had yesterday through the patient portal. I am described as a heavy smoking Native American. I am neither.

@PickyPicky3 Seems like putting the wrong results and comments on a patient could be a dangerous mistake. I'd question the test results too.

Doc came in exam room and started admonishing DH because his A1C had increased. We both sat there in stunned silence. You mean DH has diabetes? Doc must have noticed the bewildered looks on our faces and rechecked the report. Someone else's result had been stapled to DH's info. DH's blood results were just fine.

@PickyPicky3 Can you submit anything online to correct it? I have found with these patient portals, that medical staff is much more responsive as it's a documentation of their error.. As much as some people hate online portals, it makes staff more accountable.

I remember after my BF had surgery, either a radiology or surgical report mentioned that his liver was nicked as one of the diagnoses............fairly soon an "amended report" was posted.

Two of my doctors had another patient with the EXACT SAME NAME as mine. I had to make appointments using my SS number.

As a retired Certified Healthcare Documentation Specialist (fancy certification name for medical transcriptionist), I can tell you that indeed you can get your record corrected. I was once given a diagnosis of arthritis just because I was given a prescription for meloxicam which is usually given for arthritis but had been given to me for a heel deformity/pain. The assistant typed in "patient denies arthritis" and I said very loudly that "NO, patient states this is an ERROR in her medical record." Got my point across.

I can guarantee that most of you have ERRORS in your medical record since voice recognition (or as we in the profession call it, voice wreck) took over transcription. I can also guarantee the physicians do not read their notes before signing them. They didn't when I transcribed because they trusted us to get it right. We have critical thinking whereas VR does not. We know the difference between sound alike medications whereas VR does not. Medical records in the last 8 years or so are c*r*a*p.

So my cyber friends, check your medical records and correct them.

I think that is why offices also ask for a birthdate when patients call to make sure they have the right person.

They don't even pay attention. I'm 67 and was asked if I could be pregnant. I said if I was I'm suing someone because I had my tubes tied when I was 32 and if I was pregnant now I would be a medical anomaly.

I am 59 years old and when I go to a doctor I am still asked about my cycle. Unless they are talking about a bicycle there is no more cycle.

Doctors and medical providers are in hurry. They have you in and out in less than five minutes.

Last summer I was having a problem with my wrist. I was so painful I went to see my orthopedic doctor and he gave me a steroid injection.

About two weeks later, my right knee was hurting, I called for an appointment, but had to see a different doctor. This doctor whom I have never seen before without warning me, gave me an injection of steroids in both knees and only one knee was causing me problems.

I was shocked. On my way out be put his arms around my shoulders and said "you're not diabetic, are you?"

I was livid. I wish I would have told him I was. I am not. I had three steroid injection within two weeks. If I was a diabetic, he could have harmed me. I should have made him squirm.

I don't think half of the providers even read the charts. They are just in a hurry. I have also found errors on my patient portals.

Mostly info that the doctors said they went over with me and no mention was ever made to me. They are trying to cover their butt.

Unless I see something major in my portal, I just let it go. if it cause me health problems, I will get it corrected.

You know I had some goofy information in my portal and found out no one will make an effort to change it. I think it was about being treated for kidney stones - must have been the patient before me!

Having worked in the medical field for over 20 years (before computers) there were a lot less mistakes. Were there mistakes, yes but they were caught quickly and usually before the patient was contacted.

Health care workers are human and do make errors. But with todays medical corporations relying on computers there are a lot more error's.

You have to hyper vigilant and watch your records like a hawk.

I wouldn't consider this a multi-tasking error unless I knew the person entering the information was multi-tasking at the time they were entering it.

i don't multi-task well at all. I'm easily distracted or I get laser focused. I often have the TV on in the background when I'm working on the computer, but even if I planned to watch the program, I still end up missing a lot of it because I get focused on a problem I'm trying to solve.

Horrible nightmare stories. It seems like we really have to be our own advocates, can't trust the medical professionals it would seem.
