It varies, some young women have old spirits....so they can be called "old lady"....while some older ladies (65+) have young sprits...and you would NEVER EVER call them an OLD LADY...

I don't call anyone an old lady, lol! There is such a negative connotation that is attached to it.

I have known people that were old at age 20 and some that are in their 80's that are very young at heart.
Of course, when I was 20, I thought that 50 was old.
